Prevention and Wellness in Practice
In my practice, I am adding new features that will better enable me to help you
be as healthy as possible. These tools will help me to know you better and prevent
little things from becoming big problems. The new features include:

In addition to the screenings and tests that are performed, I will typically spend
over an hour with you performing the exam and discussing your test results and wellness
plan. This enhanced physical will typically include the following:*

Personal Health Record on CD and online

Be sure to carry your mini-CD in your wallet in case of an emergency. Your health
records on the CD can also be accessed through the internet via your secure, personal
website. Information includes:
— Personal Identification

— Past Medical, Social & Family History
— Medications
— Physical Exam Report
— Laboratory Test Results
— EKG

Login to your personal website to view the same information contained on your CD
and to access secure messaging so we can communicate privately about any health concerns you may have.

In order to have time to implement these new prevention and wellness tools, I will
limit the number of patients I see to a maximum of 600. This smaller practice size
will allow me to be there for you when you need me and to provide:
• Same-day or next-day appointments that start on time
• Unhurried office visits that last as long as you need
• After-hours availability by phone or pager, if needed
Additionally, a smaller practice will enable an enhanced level of care:
• We’ll have more time for questions, in-depth discussions and the opportunity to consider new treatment options for chronic conditions.
• I will be able to offer an enhanced level of coordination for specialist care and second opinions through MDVIP’s Medical Centers of Excellence Program.

Costs and Insurance
What am I getting for my money? The annual fee for joining Dr. _______’s new practice is $1500/$1650/$1800 per adult.
The fee covers your comprehensive annual physical — like those given to executives of many large corporations — at a much lower cost. You also receive a mini-CD of your health records to carry with you, and a personal website with your health information and wellness tools. In addition, your doctor will follow up with a wellness plan and work with you throughout the year on keeping you at your healthiest. Other benefits will come from your doctor’s smaller practice size which will enable him to be more available and more focused on you.
For this kind of care, and the peace of mind that comes from having a great physician who really knows you, the cost is only about $125 per month — for health care that you can’t get anywhere else.
How does my insurance work in an MDVIP-affiliated practice? Your annual fee is for preventive care services that are generally not covered by insurance. You will still need health insurance or Medicare to cover healthcare visits unrelated to your annual preventive care examination, including office visits, hospital stays and visits to specialists.
