What is the relationship between my doctor
and MDVIP?
|
|
|
MDVIP provides the services required to assist your doctor in providing a focus
on preventive care and personalized service that is not possible in a traditional
primary care practice. Your doctor owns and manages the practice, and MDVIP has
absolutely no involvement in any medical decisions. MDVIP's job is to simply remove
all obstacles that exist in the delivery of the highest level of care. MDVIP provides
comprehensive research, technological and operational support for the new practice.
We also provide insurance and regulatory services. Additionally, we conduct regular
quality assurance surveys to assist your doctor in ensuring that the practice exceeds
your expectations.

Costs, Insurance and Medicare

Is Medicare accepted?
|
|
|
Yes. MDVIP-affiliated practices are compatible with Medicare. Your annual fee covers
those services not reimbursed by Medicare. Your Medicare Part B coverage will continue
to work with your affiliated physician as it currently does.
|

How does my insurance work in an MDVIP-affiliated
practice?
|
|
|
Most PPO, POS and HMO health insurance plans are compatible with your membership
in an MDVIP-affiliated practice. We will be happy to discuss your specific health
plan with you. Your annual fee is for preventive care services that are generally
not covered by insurance. You will still need health insurance to cover your healthcare
visits that are unrelated to the comprehensive, annual preventive care examination,
including office visits, hospital stays and visits to specialists.

My insurance already provides for an annual
physical. What is different
|
|
|
about the MDVIP exam?
|
|
|
MDVIP-affiliated physicians provide an annual preventive care physical examination
that is much more thorough than the kind that is typically covered by insurance.
During your MDVIP physical, you will typically spend more than an hour with your
physician. After the physical examination, you will work with your physician on
a personal wellness plan to be implemented throughout the year. You will also receive
a Personal Health Record mini-CD containing the results of your physical examination,
lab tests, EKG, summary of medical history and emergency contact information. The
mini-CD is the size of a credit card and designed to be conveniently carried in
your wallet to be available in case of emergencies.

Does my insurance cover the annual fee?
|
|
|
No. Most insurers view the preventive care services encompassed by the annual fee
as non-covered services and do not provide reimbursement for the annual fee. Nonetheless,
if your physician is out-of-network and you have out-of-network benefits, some insurers
may cover a small portion of the annual fee. Insurance plans vary so check with
your physician at the time of your physical examination.

Do my corporate benefits cover the annual
fee?
|
|
|
The annual fee may be paid through most employer Section 125 plans and is compatible
with most flexible spending accounts (FSAs), medical savings accounts (MSAs), and
health reimbursement accounts (HRAs). The fee may also be paid through health savings
accounts (HSAs). Your physician will provide you with a statement for your benefits
administrator at the time of your physical examination or any time thereafter. It
is important to note that while this fee is reimbursable through your FSA, you cannot
pay the fee with your FSA debit or credit card. Reimbursement for your annual fee
will have to be obtained through your FSA claim submission process. Your annual
physical must be performed during your FSA plan year to be eligible for reimbursement.
For example, if your FSA account year runs from January 1, 2009 through December
31, 2009, you must have your annual physical during the 2009 calendar year.

Do I have to pay the annual fee all at once?
|
|
|
No. The annual fee may be paid quarterly, semi-annually or annually. Should you
be interested in the quarterly payment plan, you may use your American Express,
MasterCard, Visa, or Discover Card. Flexible spending account debit or credit cards
are not compatible.
|
|
Is the annual fee tax deductible?
|
|
|
For patients who itemize medical expenses on their tax returns, the annual fee may
be deductible. MDVIP representatives are available to speak with your accountant.
|
|
Can I get a refund if I am not satisfied?
|
|
|
If you have not received your physical examination, you can receive a prorated refund.
If you have received your physical, a refund is not available. Given that more than
90 percent of patients in MDVIP-affiliated practices around the country renew each
year, we believe you will be very satisfied with the MDVIP-affiliated practice.

|
Children, Family and Visitors
|
|
Do I have to pay for my children?
|
|
|
As a courtesy to their patients, most MDVIP-affiliated physicians will see patients’
children between the ages of 16 to 25. This policy may vary, so please check with
your MDVIP-affiliated physician. The only charge will be for the regular office
visit fee.
|
|
I have family members who often visit me.
Can my doctor see them
|
|
|
if needed?
|
|
|
Yes. In the event you have an out-of-town guest who may require services on an unexpected
basis, your doctor will see your guest for urgent matters as if he/she were a member
of the practice. Standard office visit fees apply.
|
|
|
How Does MDVIP Work?
|
|
What happens if more than 600 people want
to join?
|
|
|
Membership is based on a first-come, first-served basis. When a practice reaches
600 and additional patients would like to join, a waiting list is established. If
a specific practice is full, MDVIP will make every effort to place you in another
excellent MDVIP-affiliated practice in your area.
|
|
What happens when my doctor is away on
vacation or otherwise
|
|
|
unavailable?
|
|
|
Your doctor will arrange for appropriate coverage. The covering physician will provide
the same level of care and responsiveness that you will be accustomed to receiving
from your affiliated physician. You will receive contact information for the covering
physician as needed.
|
|
If I don’t join, what do I need to do?
|
|
|
If you decide not to join, please notify your doctor’s office or call MDVIP at
1-866-696-3847 as soon as possible and also complete a medical record
release form. Your doctor
will work with you to identify an appropriate physician for you. Continuity of care
is a priority for MDVIP and your physician.

|
Specialists
|
|
How does this program work with specialists?
|
|
|
MDVIP-affiliated physicians offer dramatically enhanced coordination of specialty
care. Our affiliated physicians have commented that specialists are far more eager
to care for patients who have primary care physicians who are as involved in care
decisions as your doctor will be. With this program, your doctor’s staff will have
the time to make sure that all appropriate information is given to the specialist
so that your consult is handled in the most efficient and effective manner possible.
In some cases, your doctor will personally discuss your situation with the specialist.
This makes a tremendous difference in terms of your care, as your doctor can now
serve as the “quarterback” for all of your specialist needs, with knowledge of all
of your treatment issues.

MCE
|
|
How do I use the Medical Centers of Excellence
(MCE) program?
|
|
|
Once you and your physician have determined that a second medical opinion or treatment
at an MCE facility is desired, your physician will work with MDVIP to facilitate
a timely appointment with the appropriate specialist and arrange for the transfer
of your medical records. The medical facility will then contact you directly to
arrange your appointment and discuss the personalized services you will receive
upon arrival and throughout your stay.

What happens after my visit to an MCE facility?
|
|
|
After your visit to the MCE facility, your MDVIP-affiliated physician will confer
directly with the medical center specialists. MDVIP will ensure the timely receipt
of your results and medical records by your MDVIP-affiliated physician for seamless
coordination of care.
|
|
Is there an additional cost for the MCE
program?
|
|
|
No. The cost for the actual medical treatment you received can be billed to your
insurance, depending on your healthcare coverage. Costs of travel, car service and
accommodations are your responsibility.
|
|
